Transcripts and Educational Records Requests

Transcript & Educational Records Ordering Options:

Option 1:  Online Requests

Please complete the fields in this online form.

Note: 

  1. Please be prepared to upload a photocopy of your state issued identification and a proof of name change, if applicable. 
  2. You will be provided a link at the end of completing the online form which takes you to another site to pay for your transcript/records. Unpaid requests will not be processed.

Option 2:  In Person

Note: Only the student may request his/her/their transcript when ordering in person. Please be prepared to present a photo identification [in person] when ordering a transcript.

  • Fill out the Transcript Request Form (see attached form below)
  • Pay fees (see fee schedule below)
  • Allow 3 - 5 business days for processing

Transcript & Student Record Fees:

  • High School transcripts: $10.00
  • Vocational Nurse Transcripts: $10.00
  • Attendance and Proof of Enrollment Letters: $10.00

 

Transcript requests require three (3) - five (5) business days for processing.

Duplicate High School Diplomas

Lynwood Community Adult School does not issue duplicate copies of high school diplomas. 


If you wish to obtain a duplicate copy of your diploma, please contact Herff Jones at (800) 635-5670.

  • You will need an official copy of your transcript for Herff Jones to process your order. 
  • To request an official high school transcript, please submit an online request or visit our office in person.
  • Please allow 3-5 business days for processing your request from the date your request is received by LCAS.
  • Requests are processed on a first come, first served basis. We do not offer same day or rush service.

Tips for Ordering from Herff Jones

  • Processing Time - 6- 8 weeks
  • Order must include the following items:
    • $45 money order
    • Official Transcript
    • Herff Jones Order Form (PDF file available to download below)
